For purchasing Academic Regalia upon successfully completing the Course and having applied to graduate
This form is to be completed on behalf of the deceased student by a member of the student's family, the Dean/Director of Academic Unit responsible for the program or by an Administrative Officer responsible for processing the Award
To replace Testamur with the reasons:
1) Lost or destroyed Testamur
2) Replacement Testamur showing name change
To be requested by current and past student for Swinburne to provide the proof of current and past enrolment
To amend student's personal details (if there are changes)
